School Of Basic Midwifery, Adazi Nnukwu 2016/2017 Entrance Admission Form On Sale, See More Info
This is to inform the general public that the 2016/2017 Admission Form for Entrance into the School of Basic Midwifery, Adazi Nnukwu is now on sale.
Requirements:
Candidates must have CREDIT in WAEC/NECO in the following subjects in not more than two sittings:
a. English Language
b. Mathematics
c. Biology
d. Chemistry
e. Physics.
Method of Application:
Admission form is obtained from the School on payment of a non-refundable fee of five thousand, four hundred naira only (N5, 400).
Submission:
The Entrance form is submitted to the School with the following:
a. Two (2) coloured passport photographs.
b. Birth Certificate/Declaration of Age.
c. Photocopy of WAEC/NECO Certificate or Computer Print-out.
d. Testimonial (Secondary School)
e. NECO/WAEC Scratch card (s) as the case may be.
Entrance Examination:
The entrance examination is scheduled as follows:

Time: 8am prompt
